Adult Services:
  
Community Support Team (CST)
 
Available to adults only and  is most suitable for adults who are affected by 4 or more of the following conditions:
 
Co-diagnosis of substance abuse and mental illness
 
High use of acute psychiatric hospitals/crisis/emergency services
 
History of inadequate follow through
 
Difficulty with medication management
 
Legal issues
 
Homeless or high risk for homelessness
 
Suicidal gestures or thoughts in the past 3 months
 
Ongoing inappropriate public behavior in the past 3 months
 
Self-harm or threats of harm to others in the past year
 
Evidence of significant complications such as cognitive impairment, behavioral problems, or medical conditions
 
Lower level of care has been tried or considered and found to be inappropriate either in the past or at this time
 
 
Services are delivered utilizing a team approach.  The team will work with the individual receiving service to achieve expected outcomes of decreased crisis episodes and increased ability to remain stable and functional in the community, increase in social contact, as well as personal satisfaction and independence.  Through supports based on the individual's needs, persons receiving service will reside in independent or semi-independent living arrangements, and be engaged in the recovery process.
 
Assertive Community Treatment Team (ACTT)
 
ACTT is an evidence-based psychiatric rehabilitation practice that provides a comprehensive approach to service delivery to consumers with severe mental illness.  ACTT uses a multidisciplinary team, which typically includes a psychiatrist, a nurse, and at least two case managers.  ACTT is characterized by 1) low client to staff ratio; 2) providing services in the community rather than in the office 3) shared caseloads among team members; 4) 24-hr staff availability; 5) direct provision of all services by the team (rather than referring consumers to other agencies).  Cases requiring ACTT intervention are more intensive than  CST.  A service recipient is referred to ACTT services when it has been determined that his/her needs are so pervasive and/or unpredictable that they cannot be met effectively by any other combination of available community services.  Service objectives are designed to promote symptom stability and appropriate use of medication, maintenance of physical health, restoration of personal community living and social skills, establishment of access to entitlements, housing, work, and social opportunities, and promotion of maintaining the highest level of functioning in the community.
